    A school nurse advances the well-being, academic success and lifelong achievement and health of students by:   
    • Providing urgent care with a focus on prevention and wellness.
    • Serving as a medical liaison between the school, students, families and health care providers.
    • Overseeing state-mandated on-site vision and hearing screening.
    • Monitoring student immunization status.

    Does your student need to take medication at school (every day, short term prescription, or as needed)? Complete the Authorization for Medications to be Given at School form (parent/guardian completes/dates/signs top half; healthcare provider completes/dates/signs lower half; fax or bring to school).
